The Job Description

General Statement of Duties
Performs difficult, advanced technical work providing planning and supervision in support of two Care Management Programs: Care Management for High Risk Pregnancy (CMHRP) and Care Management for At-Risk Children (CMARC), and the Post-Partum Newborn Home Visiting program.  

Distinguishing Features of the Class
An employee in this position acts as the Pregnancy Care Manager providing case management for a designated caseload of pregnant women, with sometimes-complicated pregnancies or critical environmental concerns. This position will work with clients and other community and health care providers to facilitate client access to health and social services programs which enhance the patient’s health/wellbeing and her ability to adhere to the prescribed medical treatment regimen in the most efficient and cost-effective manner possible. This position is a supervisor level role, providing professional nursing work in primary, preventive, and rehabilitative care management to individuals and families in the assigned programs. This position oversees schedules, time records, leave requests for employee time and participates in performance evaluations and other quality assurance activities. The employee may participate in program or agency-wide planning and quality assurance activities; contribute observations or data related to patient or community trends or practices ideas for improved program performance; and participate in record audits and other program reviews. This position assesses program guideline adherence, program performance, and measures program adherence with program objectives and meeting program outcomes and benchmarks. Work is performed in clinical and community settings under the direct supervision of the Public Health Nursing Director I, and is evaluated by review of records, reports, in conferences, approval of seminars and observation.

Duties and Responsibilities
Essential Duties and Tasks
Provides direct supervision of staff assigned to the CMHRP, CMARC, and Post-Partum Newborn Home Visiting programs. 
Develops program goals, policies and procedures, and quality assurance activities to meet program requirements, benchmarks and outcomes.
Meets with community agencies and medical providers to ensure program outreach and collaboration.
Assesses employee performance, approves time off requests, and manages scheduling of staff.
Uses professional case management skills to provide Case Management services to an assigned caseload.
Completes initial patient assessment and identifies problems, needs, and concerns.
Accurately identifies and prioritizes patient problems using critical thinking skills.
Collaborates with client, physician, and other providers to develop a comprehensive plan of care. Assists clients in developing personal goals.
Schedules encounter sessions in collaboration with the health care team and informs patient of the encounter schedule.  
Effectively communicates plan of care and other patient related activities both verbally and in writing to appropriate personnel and agencies.
Utilizes clinical skills and assessments to work autonomously in outpatient, community settings, and to communicate with physicians and other providers regarding alterations to plan of care.
Demonstrates the ability to use teaching, learning and counseling skills.
Completes referrals, acts as a liaison with hospital, physicians, and other programs to locate qualified patients for this program.  

Additional Job Duties
Any other duty as necessary, including assisting with any public health emergency including but not limited to natural disasters, man-made disasters, influenza pandemics or other outbreaks of communicable diseases, disaster surveillance and response to ensure efficiency of the Health Department.

Physical Requirements
Work frequently requires the exertion of up to 25 pounds of force and infrequently requires the exertion of up to 50 pounds of force.
Work frequently requires sitting, speaking or hearing, using hands to finger, handle or feel and reaching with hands and arms, pushing or pulling and repetitive motions and regularly requires standing, walking, climbing or balancing, and stooping, kneeling, crouching or crawling.
Work requires close vision, distance vision, depth perception, color perception, and peripheral vision.
Vocal communication is required for expressing or exchanging ideas by means of the spoken word and detailed and loud talking to convey detailed or important spoken instructions to others accurately, loudly, or quickly.
Hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound.
Work requires preparing and analyzing written or computer data, operating equipment, and observing general surroundings and activities.
Work requires regular exposure to blood borne pathogens and may be required to wear specialized personal protective equipment.
Work is generally in a moderately noisy location (e.g. business office, light traffic).

Desirable Education and Experience
Graduation from a college or university with a Bachelor’s Degree in Nursing, which includes a Public Health Nursing rotation and one year of Public Health Nursing experience; or an equivalent combination of training and experience.   

Additional Requirements
Possession of a current CPR Certification upon hire.
Possession of a valid license to practice nursing form the North Carolina State Board of Nursing upon hire.
